ABSTRACT

Valid and reliable measures are necessary to provide person-centered care. However, there are no such measures in Korean nursing home (NH) settings. The purpose of the current study was to translate and culturally adapt the Preferences for Everyday Living Inventory (PELI) into Korean following the International Society for Pharmacoeconomics and Outcomes Research guidelines. Face validity was assessed by cognitively capable Korean older adults (N = 10) using a 4-point Likert scale (1 = strongly disagree to 4 = strongly agree). Mean score was 2.7 (SD = 0.82) for grammar and wording; 3.7 (SD = 0.48) for understandability; and 3.7 (SD = 0.67) for cultural relevance. Participants found the Korean version of the PELI (PELI-K) easy to understand and interpret, and culturally relevant. Results suggest that the PELI was successfully translated and culturally adapted to Korean. Implementing the PELI-K in Korean NHs and incorporating individual preferences into care delivery will improve quality of care. [Journal of Gerontological Nursing, 49(5), 5-10.].

ABSTRACT

Brain health refers to the state of a person's brain function across various domains, including cognitive, behavioral and motor functions. Healthy brains are associated with better individual health, increased creativity, and enhanced productivity. A person's brain health is intricately connected to personal, social and environmental factors. Racial, ethnic, and social disparities affect brain health and on the global scale these disparities within and between regions present a hurdle to brain health. To overcome global disparities, greater collaboration between practitioners and healthcare providers and the people they serve is essential. This requires cultural humility driven by empathy. Empathy is a core prosocial value, a cognitive-emotional skill that helps us understand ourselves and others. This position paper aims to provide an overview of the vital roles of empathy, cooperation, and interdisciplinary partnerships. By consciously integrating this understanding in practice, leaders can better position themselves to address the diverse challenges faced by communities, promote inclusivity in policies and practices, and further more equitable solutions to the problem of global brain health.

ABSTRACT

Aging accelerates during midlife. Researches have shown the health benefits of mind-body intervention (MBI). However, whether MBI is involved with aging process has not been well understood. In this study, we approach to examine the relations of MBI with this process by investigating an aging marker of the peripheral blood, blood chemistry, and self-report questionnaires. A quasi-experimental design was applied. Experienced MBI practitioners participated in a 3-month intensive meditation training, while the age, gender-matched MBI-naïve controls led a normal daily life. Measurements were taken at before and after the 3 months for relative telomere length (RTL), blood chemistry, and self-report questionnaires including items about sleep quality, somatic symptoms, depression, anxiety, stress, emotional intelligence (EI), and self-regulation. For RTL, the repeated measures analysis of variance showed a significant group*time interaction (P = .013) with a significant post hoc result (P = .030) within the control group: RTL was significantly reduced in the control while it was maintained in the meditation group. In repeated measures analysis of variance for blood chemistries, there were significant group differences between the groups in glucose and total protein. In the post hoc comparison analysis, at post measurements, the meditation group exhibited significantly lower values than the control group in both glucose and total protein. There were significant group-wise differences in the correlations of RTL with triglyceride (TG), high-density lipoprotein (HDL), glutamic oxaloacetic transaminase and glutamic pyruvic transaminase. Any of self-report results did not show significant changes in group*time interaction. However, there were group differences with significant (P < .05) or a tendency (.05 < P < .1) level. There were significant improvements in depression, stress and EI as well as tendencies of improvement in sleep quality and anxiety, in the meditation group compared to the control group. Our results suggest that meditation practice may have a potential to modify aging process in molecular cellular level combined with changes in psychological dimension.

ABSTRACT

The importance of social networking and the online environment as core factors in building relationships has grown as a result of the COVID-19 pandemic, which limited in-person activities. As classes transitioned to online platforms, there was an influx of elementary school students into the cyberspace, increasing the risk of exposure to cyberbullying. This study analyzed the factors influencing the experience of cyberbullying among Korean elementary school students around 2020, when the spread of COVID-19 began in earnest, and thus suggests directions for cyberbullying prevention measures for the post-COVID-19 era. This comparative study used binary logistic regression to analyze data from the "Cyber Violence Survey" conducted by the Korea Communications Commission in 2019 and 2020. The analysis confirmed that interactions between parents and children, cyberbullying control by schools, and recognition of cyberbullying as a problem had statistically significant influences on cyberbullying experience only in 2020 (i.e., when the pandemic began). Overall, this study emphasizes the importance of raising awareness about cyberbullying among elementary school students and taking preventive action through a home-school system to address cyberbullying in the post-COVID-19 era.

OBJECTIVE: Amniotic mesenchymal stromal cells (AMSCs) can be obtained from the mesoderm of human amniotic membrane. AMSCs derived from term baby show increased expression of genes associated with apoptosis and senescence. The objective of this study was to examine gene expression profiles of AMSCs derived from preterm (preterm AMSCs) and term labors (term AMSCs) and analyze common and different mechanisms. MATERIALS AND METHODS: We isolated and cultured AMSCs from 43 placentas: 27 from term labor and 16 from preterm labor. Microarray analysis and gene network analysis were performed to compare gene expression profile (GEP) of preterm (n = 6) with term AMSCs (n = 10). Senescence-associated gene (CDKN2A and CDKN2B) expression was also measured by reverse transcription quantitative PCR. RESULTS: GEP demonstrated that preterm AMSCs showed upregulation of nicotinamide adenine dinucleotide biosynthetic process and downregulation of extracellular matrix, cholesterol import and transport, lipid storage, and maintenance of location. CDKN2A and CDKN2B genes showed similar expression levels between term and preterm AMSCs. CDKN2A gene expression was correlated with CDKN2B expression and population doubling time. Compared to term AMSCs, preterm AMSCs showed significantly different expression of genes associated with inflammatory response which could be one of the major players in labor events. CONCLUSION: Increased CDKN2A expression in AMSCs is associated with placental membrane aging which participates in both preterm and term labor. To the best of our knowledge, this is the first report to demonstrate the association of AMSCs with labor.

ABSTRACT

Because of the implementation of social distancing measures following the onset of the COVID-19 pandemic, face-to-face interaction has plummeted, which has resulted in the prolific use of social networking services (SNS) and increased activity in the cyberspace environment. This is especially true for teenagers and young adults with the shift to online classes in the education sector, which has increased the chances of being exposed to cyberbullying. This study attempts to determine a strategy for counteracting cyberbullying in the post-COVID-19 era by identifying the factors that have contributed toward greater aggression by adolescents in South Korea in 2020 when the spread of COVID-19 was at its height. To achieve this, we employed the Cyberbullying Circumstance Analysis dataset from the Korea Communications Commission for the time frame of between 2019 and 2020, with 4779 and 4958 participants, respectively. The causes and effects that led to cyberbullying were investigated using binary logistic regression analysis. By reviewing the research data targeted towards Korean adolescents, our analysis found that the average age of those who engage in cyberbullying decreased in 2020 compared to 2019. In addition, cyberbullying awareness at school and the school's capability of controlling it influenced the number of cyberbullies from a statistical grounding, which could be found only in the 2020 dataset. An approach to establishing policies in schools to contain preemptive measures and increase cyberbullying awareness has been proposed to prevent such mishaps in the post-COVID-19 era.

ABSTRACT

The current study aimed to assess relationships among role conflict, nursing professionalism, nursing work environment, and retention intention of nurses at long-term care (LTC) hospitals. Participants comprised 183 nurses at nine LTC hospitals located in Busan, Korea. Data were collected using a structured self-report questionnaire and analyzed using descriptive statistics, t test, one-way analysis of variance, Pearson's correlation coefficient, and multiple linear regression. Retention intention had a signifi-cant positive correlation with nursing professionalism (r = 0.39, p < 0.001) and nursing work environment (r = 0.51, p < 0.001). Nursing work environment had a significant negative correlation with role conflict (r = -0.30, p < 0.001) and a significant positive correlation with nursing professionalism (r = 0.48, p < 0.001). In the final multiple regression analysis, factors influencing retention intention of participants included number of beds (ß = -0.15 p < 0.026), nursing professionalism (ß = 0.19, p = 0.007), and nursing work environment (ß = 0.36, p < 0.001). The explanatory power of number of beds, nursing professionalism, and nursing work environment on retention intention was 34% (F = 16.66, p < 0.001). Improving nursing professionalism and nursing work environment of nurses at LTC hospitals will ultimately enhance their retention intention and positively impact the quality of gerontological nursing care. ABSTRACT

The crystallization kinetics in BaTiO3 synthesis from hydrate precursors via microwave-assisted heating (MWH) were investigated. The structural and chemical features of powders synthesized via MWH and conventional heating (CH) were compared. The charged radicals generated under microwave irradiation were identified by chemical analysis and real-time charge flux measurements. Using Ba(OH)2∙H2O (BH1), Ba(OH)2 (BH0), and BaCO3 (BC) as the precursors for a Ba source, and TiO2∙4H2O (TH) for a Ti source, three different mixture samples, BH1TH (BH1 + TH), BH0TH (BH0 + TH), and BCTH (BC + TH), were heat-treated in the temperature range of 100-900 °C. BaTiO3 powders were synthesized at temperatures as low as 100 °C when sample BH1TH was subjected to MWH. Based on the growth exponent (n), the synthesis reactions were inferred to be diffusion-controlled processes (3 ≤ n ≤ 4) for MWH and interface-controlled processes (2 ≤ n ≤ 3) for CH. Current densities of approximately 0.073 and 0.022 mA/m2 were measured for samples BH1TH and BH0TH, respectively, indicating the generation of charged radicals by the interaction between the precursors and injected microwaves. The radicals were determined as OH- groups by X-ray photoelectron spectroscopy and Fourier transform infrared spectroscopy.

INTRODUCTION: The single most common cause of miscarriage is genetic abnormality. OBJECTIVE: We conducted a prospective cohort study to compare the performance of conventional karyotyping and chromosomal microarray analysis (CMA) using array comparative genomic hybridization (array-CGH) and single nucleotide polymorphism array (SNP-array) to identify genetic abnormalities in miscarriage specimens. METHODS: A total of 63 miscarriage specimens were included. Conventional karyotyping, array-CGH, and SNP-array were performed and the results compared. RESULTS: Genetic abnormalities were detected in 31 cases (49.2%) by at least one testing modality. Single autosomal trisomy was the most common defect (71.0%), followed by polyploidy (16.1%), multiple aneuploidy (9.7%), and monosomy X (3.2%). Mosaicisms were identified in four cases and confirmed by fluorescence in situ hybridization (FISH) using appropriate probes. SNP-array had a higher detection rate of genetic abnormalities than array-CGH (93.5 vs. 77.4%), and conventional karyotyping had the lowest detection rate (76.0%). SNP-array enabled the detection of all types of genetic abnormalities, including polyploidy. CONCLUSIONS: Although conventional karyotyping and FISH are still needed, SNP-array represents the first choice for miscarriage because the technique showed excellent performance in the detection of genetic abnormalities and minimized the probability of testing failure as well as time, costs, and labor.

ABSTRACT

The anomalous connection of umbilical vessels to the heart is rare and has not yet been reported in the international scientific literature. Herein, we report the case of a newborn who was diagnosed with an anomalous connection of the umbilical vessels to the left ventricle. These anomalous vessels were functionally open for 2 weeks, and cellulitis was present in the area of the blood vessels connected to the skin. We performed division of these abnormal vessels and removal of the skin lesion.

ABSTRACT

BACKGROUND: Dienogest has been shown to substantially improve endometriosis-associated symptoms such as debilitating chronic pelvic pain, and in turn, health-related quality of life (HRQoL). To date, there is no data on patient-reported outcomes reflecting the real-world practice in Asia where endometriosis is a relevant health, social and economic burden. This non-interventional, multi-center, prospective study aims to investigate the influence of dienogest on HRQoL. METHODS: Asian women received dienogest (2 mg/daily) and were followed for 24 months. The effectiveness of dienogest to improve HRQoL and endometriosis-associated pelvic pain (EAPP) was assessed by patient-reported outcomes. HRQoL, especially the "pain" domain as primary endpoint, was evaluated with the Endometriosis Health Profile-30 (EHP-30) questionnaire. The numeric rating scale served to determine changes in the severity of EAPP. Within the presented interim analysis (data cut-off: 2017-11-27), the mean changes in EHP-30 and EAPP scores from baseline to 6 months upon availability of the data were evaluated. Treatment-emergent adverse events (TEAEs) and bleeding profiles were documented. RESULTS: Dienogest therapy decreased EHP-30 scores in all assessed domains (score 0-100, lower scores indicate better HRQoL). Primarily, the "pain" domain was improved in 78.4% of patients. EAPP was reduced (score 0-10, lower scores reflect less pain), highlighted by a mean reduction of the pain score by - 4.5 points. Patients with a higher EAPP score at baseline had an increased response to dienogest (- 6.2 points mean change) compared to patients with low baseline EAPP severity (- 1.4 points mean change). Both surgically and clinically diagnosed patients described comparable pain reduction, as well as women with or without prior treatment. Drug-related TEAEs were documented for 31.5% of patients, with amenorrhoea (5.9%) and metrorrhagia (5.1%) being the most common events. The bleeding pattern was changed upon dienogest, characterized by decreased normal bleeding (84.2 to 28.8%) and increased amenorrhea (3.2 to 42.9%) at 6 months. CONCLUSION: The data indicate an amelioration of HRQoL and EAPP upon dienogest therapy. No new safety signals were observed. Therefore, its use as first-line therapy for long-term management of debilitating and chronic endometriosis-associated pain represents an interesting option that remains to be further investigated. ABSTRACT

PURPOSE: The aim of this study was to understand the concept of innovative behavior and its theoretical and practical implications for nursing. METHODS: Eight-step modification of Wilson's classic concept analysis procedure (1963) by Walker and Avant's (2005) was used to explore the antecedents, attributes, and consequences of innovative behavior in the literature. Electronic searches were conducted in PubMed, Google Scholar, OVID Medline, Science Direct, and ERIC databases using "innovative behavior" or "innovative work behavior", "innovativeness", "innovation", "leadership", "healthcare", and "nursing" as keywords, with no limitation on publication date. RESULTS: Organizing the framework based on the method of concept analysis by Walker and Avant , defining attributes to innovative behavior were opportunity exploration, idea generation, idea search, idea communication, promotion of idea, idea championing, application, and overcoming obstacles. Antecedents to innovative behavior are categorized into three groups: organizational characteristics, work environmental characteristics, and individual characteristics. Consequences of innovative behavior included job productivity, lower levels of job burnout, job satisfaction, solving the organizational problems, organizational commitment, organizational efficiency, and effectiveness. CONCLUSION: Eight dimensions including opportunity exploration, idea generation, idea search, idea communication, idea promotion, championing, application, and overcoming obstacles were analyzed. We suggest promoting innovative behavior through leadership and management in nursing. Future research should focus on developing instruments and conducting empirical studies on innovative behavior in nursing research and practice.

ABSTRACT

The aim of the current study was to investigate the moderating effects of resilience on the relationship between emotional labor and burnout among care workers in long-term care (LTC) hospitals. Participants were 126 care workers from five different LTC hospitals in Busan, South Korea. A set of self-reported questionnaires was administered to assess general characteristics, emotional labor, resilience, and burnout of participants. Data were analyzed using descriptive statistics, t tests, one-way analysis of variance, Pearson correlation coefficients, and hierarchical multiple regression. The final multivariate regression model was statistically significant and accounted for 36% of the variance in burnout. Emotional labor was significantly associated with burn-out (ß = 0.25, p = 0.001). Resilience had a significant moderating effect on the relationship between emotional labor and burnout (ß = -0.16, p = 0.033). To alleviate burnout in care workers, emotional labor should be recognized as a significant mental health problem and management interventions should be developed. AIM AND OBJECTIVE: This study investigated the effects of medication adherence and health literacy on health-related quality of life in vulnerable older people with hypertension. BACKGROUND: Health literacy is particularly critical for providing accurate information regarding correct medication intake to improve medication adherence. Additionally, health-related quality of life is directly related to impairment from chronic disease. DESIGN: A descriptive, cross-sectional study design was used. The participants were 160 low-income older people with hypertension registered in 16 public health centres in Busan, South Korea. They had received "visiting nursing services" for at least 6 months until the latest date and had belonged to the priority group of visiting nursing services. METHODS: The Morisky Medication Adherence Scale, the Newest Vital Signs and EuroQol-5 Dimensions were used to assess medication adherence, health literacy and health-related quality of life, respectively. RESULTS: Medication adherence and health literacy were significantly associated with health-related quality of life in vulnerable older people with hypertension, although exercise and subjective health were more significant factors affecting health-related quality of life than medical adherence and health literacy. Level of education, monthly income and employment status were not associated with health-related quality of life. CONCLUSIONS: To effectively promote health-related quality of life in this population, medication adherence and health literacy of patients should be considered when developing health interventions, including subjective health and exercise. IMPLICATIONS FOR PRACTICE: To effectively promote health-related quality of life in vulnerable older people, medication adherence and health literacy of patients should be assessed, and patient-centred intervention strategies that consider their individual differences should be developed.

ABSTRACT

This study explored the discrepancy between perceived cognitive dysfunction and computerized neuropsychological test performance in Korean patients with RA. Individuals with RA were recruited by their rheumatologists during follow-up visits at one hospital in Korea. After getting signed consents, a trained research nurse assessed participants with a range of physical, psychosocial, and biological metrics. Subjective cognitive dysfunction was assessed using the Perceived Deficits Questionnaire (PDQ; range 0-20). Objective cognitive impairment was assessed using a set of 6 computerized neurocognitive tests yielding 12 indices covering a range of cognitive domains. Subjects were classified as 'impaired' if they performed 1 SD below age-based population norms on each test. A total cognitive function score was calculated by summing the transformed scores (range 0-12). Multiple linear regression analyses determined the relationship of total cognitive function score with PDQ score, controlling for disease activity, functional limitations, and psychological factors. Forty subjects (mean ± SD age: 63.1 ± 11.4 years) were included. Mean ± SD scores of total cognitive function and PDQ were 7.3 ± 2.7 (2-12) and 11.8 ± 5.1 (5-25), respectively. In multivariate analysis, there was no significant relationship between total cognitive function score and PDQ score. However, depression and sleep quality (ß = 0.37, p = 0.025; ß = 0.17, p = 0.034) were significantly associated with PDQ score. The findings emphasize the gap between subjective and objective measures of cognitive impairment and the importance of considering psychological factors within the context of cognitive complaints in clinical settings.

BACKGROUND: In infants, oliguria is defined as a urine output of <1.5 mL/kg/h. The aim of our study was to assess the impact of oliguria on urinary neutrophil gelatinase-associated lipocalin (NGAL) and serum cystatin C (CysC) levels in very-low-birth-weight infants (VLBWIs) with a normal serum creatinine (Cr) level. METHODS: Fifty-seven VLBWIs were enrolled in the study. Urinary NGAL, serum CysC and Cr levels and urinary NGAL/Cr ratios were measured. Infants with Apgar scores of >5 at 5 min and/or a serum Cr level of >1.5 mg/dL or those treated for patent ductus arteriosus were excluded. In case of antibiotic treatment, blood and urine samples were collected at ≥48 h after discontinuation of antibiotic treatment. RESULTS: There was a significant difference in gestational age between infants with oliguric episodes during hospitalization and those without, but not in birth weight, perinatal or postnatal factors. Gestational age was negatively correlated with urinary NGAL and serum CysC levels and urinary NGAL/Cr ratio (p < 0.05), whereas postnatal age was negatively correlated with serum Cr level and urinary NGAL/Cr ratio (p < 0.05). Of the 117 urine and blood samples collected, 25 (21.4%) were obtained from neonates with oliguric episodes. After adjusting for gestational age and postnatal age, comparison of samples collected in infants with and without oliguric episodes revealed significant differences in the mean level of urinary NGAL and in the urinary NGAL/Cr ratio, but not in mean serum CysC or serum Cr levels. The urinary NGAL level [area under the curve (AUC) 0.886, 95% confidence interval (CI) 0.814-0.937] and urinary NGAL/Cr ratio (AUC 0.853, 95% CI 0.775-0.911) showed significantly greater discrimination for oliguria than serum CysC (AUC 0.610, 95% CI: 0.515-0.699) or serum Cr (AUC 0.747, 95%CI 0.659-0.823) levels. CONCLUSIONS: Urinary NGAL level and urinary NGAL/Cr ratio were more sensitive markers for the presence of oliguria in VLBWIs with normal serum Cr levels than serum CysC level.

ABSTRACT

STUDY OBJECTIVE: To compare the clinical efficacy and safety of laparoscopic cornuotomy and cornual resection in the treatment of interstitial pregnancy. DESIGN: Retrospective chart review between 2006 and 2014 (Canadian Task Force classification II-2). SETTING: Two academic tertiary care hospitals. PATIENTS: Seventy-five patients with interstitial pregnancy treated by laparoscopy. MEASUREMENT AND MAIN RESULTS: In the 75 patients, 53 who underwent cornual resection and 22 who underwent cornuotomy, we evaluated operating time, changes in hemoglobin levels after surgery, the rate of major complications, and the incidence of persistent interstitial pregnancy. The mean operating time was significantly shorter for cornuotomy than for cornual resection (59.36 ± 19.32 minutes vs. 77.11 ± 23.97 minutes, respectively). Changes in hemoglobin level after the operation, rates of major complications, and the incidence of persistent interstitial pregnancy were not significantly different in the 2 surgery groups. CONCLUSION: Laparoscopic cornuotomy yielded clinical results comparable to those of cornual resection. Laparoscopic cornuotomy may reduce the time of operation, and had the same incidence of persistent interstitial pregnancy as cornual resection.

ABSTRACT

The incidence of type 1 diabetes is increasing worldwide, and the greatest increase has been observed in very young children under 4 years of age. A case of infantile diabetic ketoacidosis in a 10-month-old male infant was encountered by these authors. The infant's fasting glucose level was 490 mg/dL, his PH was 7.13, his pCO2 was 15 mmHg, and his bicarbonate level was 5.0 mmol/L. The glycosylated hemoglobin level had increased to 9.4%. Ketonuria and glucosuria were detected in the urinalysis. The fasting C-peptide and insulin levels had decreased. The infant was positive for anti-insulin and antiglutamic acid decarboxylase antibodies. Immediately after the infant's admission, fluid therapy and intravenous insulin infusion therapy were started. On the second day of the infant's hospitalization and after fluid therapy, he recovered from his lethargic condition, and his general condition improved. Feeding was started on the third day, and he was fed a formula 5 to 7 times a day and ate rice, vegetables, and lean meat. Due to the frequent feeding, the frequency of rapid-acting insulin injection was increased from 3 times before feeding to 5 times, adjusted according to the feeding frequency. The total dose of insulin that was injected was 0.8-1.1 IU/kg/day, and the infant was discharged on the 12th day of his hospitalization. The case is presented herein with a brief review of the relevant literature.

ABSTRACT

Benign metastatic leiomyomatosis (BML) is a rare disease characterized by smooth muscle cell proliferation in extrauterine sites including the lung, abdomen, pelvis, and retroperitoneum. Depending on location, BML is classified as intravenous leiomyomatosis and diffuse peritoneal leiomyomatosis. Pathogenesis of BML can be iatrogenic after previous myomectomy or hysterectomy, hormonal, or coelomic metaplasia. Treatment options are observation, hormonal suppression, and/or surgical debulking via laparotomy or laparoscopy. Laparoscopic surgery is gaining in popularity in the gynecologic field compared to laparotomic surgery and single-port laparoscopy has the benefits of cosmesis and early tissue extraction by transumbilical morcellation. We report a 39-year-old woman with BML who underwent single-port laparoscopy debulking surgery.
